National Voter Registration Act

Initial Intake
|
| The
National Voter Registration Act requires all agencies that provide public assistance
or state-funded programs engaged in providing services to persons with disabilities,
to provide every new applicant, recertification, renewal or applicants changing
their address with a Voter Registration Opportunity Form and assistance with completing
the Form. |

1. Where and when Voter Registration must be offered:
Ideally, voter registration services should be offered as part of the
intake process. If your agency does not have a walk-in unit to assist applicant’s,
you must include a Voter Registration Opportunity Form in the application packet
and request that the response portion of the Opportunity Form be returned with
the application.
NOTE: If the applicant refuses to return the
mailed Form, you should draw a line through the response portion of a Form records

2.
Upon receipt of the application for assistance at a walk-in unit, the following
should be asked:
If you are not registered to vote where you live now, would you like to register
to vote here today? |

|
3. Hand applicant a Voter Registration Opportunity
Form and ask applicant to complete response portion of the Opportunity Form.
NOTE: If applicant cannot read, the Opportunity Form
should be read to the applicant.

| 4.
If the applicant checks the “I Am Already Registered” box,
the process is concluded. Have the applicant complete the response portion of
the Opportunity Form. |

| 5.
If the applicant checks the “No” box and wishes to sign the
Opportunity Form, you must provide assistance necessary to complete the Voter
Registration Opportunity Form and inform the applicant that the form will be retained
by the agency. |

|
|
6. If applicant "Refuses" to complete
the Opportunity Form, request response portion and draw a line across the Form
and retain for your records. Do not write applicant’s name or any other
notation on Form, unless otherwise required as part of your agency's operating
procedures. No further action is required.
NOTE: The applicant should be assured
that declining to register to vote will assistance
they will be provided and their decision will
remain confidential and will be used only for
voter registration purposes.

7.
If the applicant checks the “Yes” box, you are required
to provide applicant with a Voter Registration Application.
