The Absentee Ballot Application needs to be mailed or delivered to the County Clerks Office in the county where you reside.

Party Affiliation Declaration Form (131k pdf)
Mail or deliver the Party Affiliation Form to the Commissioner of Registration in your county or you can also file it with your municipal clerk.

Instructions: After opening the PDF, you have two options for completing the form:
A. Print out the form, fill it in by hand, and sign it. or
B. Enter your information directly into the fields on your computer screen, print out the completed form and sign it.
After completing and signing the form, mail or deliver it to your County Commissioner of Registration or Superintendent of Elections.
